Current MCD Kidz dancers can bring a friend, and new families are welcome to register for a free trial class.

Ages 5–14.

No previous dance experience required.

Motion Collective Dance, Inc. & Youth Life Center Photo/Video Release

I, as the parent or legal guardian of the participating child, give permission to Motion Collective Dance, Inc. and Youth Life Center to photograph, video record, and/or audio record my child during classes, workshops, rehearsals, performances, events, and related activities.

I understand that these photographs, videos, and recordings may be used for legitimate organizational purposes, including websites, social media, promotional materials, fundraising materials, grant applications, newsletters, advertisements, and documentation of programs and events.

I understand that my child’s image, voice, or likeness may appear in these materials and that I will not receive compensation for their use.

Motion Collective Dance, Inc. & Youth Life Center Participation and Liability Waiver

I understand that participation in dance, acrobatics, stretching, conditioning, and other physical activities involves inherent risks, including falls, collisions, strains, sprains, and other injuries.

I voluntarily authorize my child to participate in Motion Collective Dance activities held at or in partnership with Youth Life Center. I understand that Motion Collective Dance, Inc. and Youth Life Center will take reasonable precautions to provide a safe environment, but injuries may still occur during physical activity.

To the extent permitted by law, I assume the ordinary risks associated with my child’s participation and agree not to hold Motion Collective Dance, Inc., Youth Life Center, their directors, officers, employees, instructors, volunteers, or authorized representatives responsible for injuries or losses arising from the ordinary risks of participation, except to the extent caused by conduct for which liability cannot legally be waived.

If I cannot be reached in an emergency, I authorize program staff to contact emergency medical services and obtain reasonably necessary emergency care for my child. I understand that I am responsible for medical expenses incurred on my child’s behalf.

I confirm that I have disclosed any medical conditions, allergies, injuries, or physical limitations that program staff should know about before my child participates.
